Something looks wrong in my records

Who can correct a record, why ChartKey usually cannot, and how to get it fixed properly.

There are two different problems here, and they have different fixes.

If the information itself is wrong

A medication you no longer take, a condition that was ruled out, the wrong date on a procedure: that is your clinic's record, and ChartKey holds a copy of it. Correcting the copy alone would leave you with a record that disagrees with the one your clinicians actually use, which is worse than the original error.

Ask the organization that holds it. Most portals have a way to request an amendment, and their medical records department handles it. Once they correct it, the correction flows into ChartKey on a later sync.

You have a right to request a correction from the organization holding your record. If you want to note your disagreement in the meantime, add a note in ChartKey saying so, so it travels with your own timeline.

If ChartKey is displaying it wrong

Something in the right place in your portal but garbled, duplicated, or missing a value here is ours to fix. Use Send feedback from the screen where you see it. The report tells us which screen you were on; you do not need to include your health details.

If you are not sure which it is

Compare the one item with your clinic's portal. Matching there but wrong here is ours. Wrong in both places is theirs. Either way, reporting it to us costs nothing and we will tell you which it is.
