What appears on it

Visits, results, prescriptions, immunizations, procedures, documents, and the notes you add yourself. Each entry carries its date and a short line about what it was.

Finding a particular moment

Scroll to the period you have in mind, or open an entry to see its detail. If you are preparing for an appointment and want the recent picture pulled together in one place, Getting ready for a visit does that for you.

Why some dates look approximate

An item is placed using the most meaningful date the record carries: when a result was taken, when a visit happened, when a note was written. If an organization sent no date for something, it can appear at the end rather than in sequence.
